LL-37 is the only cathelicidin antimicrobial peptide produced by humans, a 37-amino-acid fragment of the hCAP-18 precursor protein, studied for direct antimicrobial membrane disruption, anti-biofilm activity, and recruitment of immune cells via formyl peptide receptor signaling.
A 2021 phase IIb randomized controlled trial in 148 patients found complete wound closure in 28.1% of large wounds treated with LL-37, versus 8.1% with placebo. In mouse studies, delivery of LL-37 to wounds in diabetic (ob/ob) mice significantly improved re-epithelialization and granulation tissue formation versus untreated wounds.
