Ipamorelin is a pentapeptide that acts as a selective agonist at the ghrelin/growth hormone secretagogue receptor, stimulating GH release from the pituitary. It was developed to trigger GH release without also raising cortisol, ACTH, or other pituitary hormones the way earlier GH-releasing peptides did.
In the original characterization study, ipamorelin released GH from rat pituitary cells and in live rats and pigs with potency comparable to an older secretagogue, while testing showed no significant rise in ACTH, cortisol, prolactin, or other pituitary hormones. Separate rat studies found that combining a GH secretagogue with glucocorticoids counteracted glucocorticoid-induced reductions in bone formation and muscle strength —animal-model findings, not human outcome data.
